Community Support Services
Our Community Support Service is partly funded by the NHS BLMK Integrated Care Board
Whether you have recently been discharged from hospital or if your acquired brain injury happened some time ago we are here to help you move forward with your life.
Who is this service for?
- Adults who have an acquired brain injury or other neurological conditions
- Families who have children with an ABI.
What does this service offer?
Support for clients, partners, carers and families include:
- Full assessment of your needs.
- Personal goal setting.
- Support following discharge from hospital/rehabilitation centre.
- Advice and information regarding acquired brain injury.
- Support for independent living.
- Advocacy and support with housing, benefits and money problems.
- Signposting to other health, legal and relevant services and agencies.
- Emotional support.
- Crisis intervention.
- Access to Centre Services aiming to improve self-confidence, personal skills, social integration.
- Referral to Luton Foodbank if needed.
What are the next steps?
- Either you, your carer, GP, health and social agencies or voluntary organisations can contact us.
- We’ll then arrange an appointment with you to discuss the next steps.
